Currently, numerous surveys are conducted with HeyForm in which not only personal data in the form of contact data but also very sensitive, statistical data, e.g. health data, are collected in surveys in the context of vaccinations against SARS-CoV-2, on the topics of vaccination readiness, appointment coordination in vaccination centers or similar. Our tool is ideally suited for these types of surveys involving personal data, as we place particular emphasis on privacy and security and take appropriate measures to protect personal data.
We are pleased that HeyForm is used for these purposes and that survey creators also trust our secure and DSGVO-compliant tool with regard to sensitive data. Here, we would like to provide a few recommendations for current and future survey creators due to a given occasion.
Recommendations for surveys that collect personal data/sensitive data:
Specify contact person for queries about the survey
Important: As the survey creator, please note a contact person for queries about the survey/query, preferably clearly visible in the introductory text of the survey and, if applicable, on the closing page.
At the end of the survey, set up a link to your own website.
In this context, we recommend setting up your own end page in the survey so those survey participants are redirected to the company or project page after completing the survey and not to the HeyForm website (as is the default). This way, in addition to providing a contact person, you ensure that your survey respondents know which institution is responsible for the survey/query. Allow participants to document their own responses
Give your survey participants the opportunity to be able to check the (personal) data entered before completing the survey and to have documentation of it. This is possible with the link to print: After clicking on the link, the survey participant can print out the questionnaire with their own answers. If necessary, it is then possible to navigate back in the questionnaire using the “back button” in the survey in order to correct information. Use this function, for example, on the penultimate page, before the closing page, e.g.: “You can use the following link to print and check your information.”
